[Scheme-reports] An R5RS erratum that persisted into R7RS

Per Richard Kelsey's R5RS errata at
<http://www.mumble.net/~kelsey/r5rs-errata.html>, the domain of `list-ref`
is off by one: `(list-ref x k)` is an error if x has k or fewer elements.
This is now on the R7RSSmallErrata page and in the "errata" branch.
