On the question of whether they should be in the base language
or a library, I would say that since car and cdr are in the base
language, and these extended forms have semantics that can be
defined in terms of them, the extended forms can be defined in
a library.
That said, I strongly suspect most implementations concerned with
speed will prefer to have them be defined as machine-code vectors,
so the library will only bind *names* for them, not provide the
implementation that actually gets used.
